MYM Participant Forms & Information
We are so delighted that you or your child will be joining us for a week of Mission Immersion in Memphis, TN. Each participant will need to complete the forms below to finalize their individual registration. We have included lots of great information so please read it carefully – we don’t want you to miss a thing or get caught unaware.
Each participant MUST complete the Participant Medical History/Release, Covenant Agreement Form (shown below and completed online).
In addition to your required forms, we have provided you with information about What to Bring, and a sample schedule for the week, as well as info about our “Welcome Home Pack” program. If you have other questions about MYM, you may want to check out our FAQs. Your Group Leader will have more information to share with you concerning your Mission Immersion week.
